New Patients Welcome

New Patients & Appointments

We’re excited to welcome you to our clinic.
To make sure we can give you the time and attention you deserve, we encourage you to schedule an appointment with one of our experienced providers. Please fill out new patient forms and return to the office prior to your visit. We do our best to see every patient promptly as possible.

Arriving on Time

Arriving about 15 minutes before your scheduled appointment gives you time to complete any necessary paperwork. Please remember to bring your current insurance card(s) to each visit.

Medications

Knowing the names of your medications helps us care for you more effectively. If you take several medications, bring a list including the dose and strength.  Or bringing the bottles to each visit would be most helpful. Please include both prescription and over‑the‑counter medications so we can best support your health.

Please allow 3-4 days for medication refills.  Most will require a prior authorization from your insurance company. 

Preparing for Your Visit

Your appointment is a great opportunity to talk with your provider about your health and any concerns you may have. Writing down your questions ahead of time can help you make the most of your visit. Some concerns may require additional time or follow up appointments. If so, the clinic will work with you to schedule another visit. Every question matters, and we’re here to help you get the answers you deserve.

Cancelling an Appointment

If you need to cancel or reschedule, please let us know 24 hours prior. This helps us offer that time to another patient who may need it. You’re welcome to call and leave a message 24 hours a day. This will also prevent a “No Show” fee.
Immunization Records

If you’re bringing your child to the clinic, please bring a copy of their immunization record. This helps us ensure they receive the most up to date information and care.

Minor Consent for Treatment

A parent or legal guardian must be present to give consent for a minor’s treatment. Some exceptions exist when minors are legally allowed to consent to certain services under state or federal law. If you’re unsure about the requirements, feel free to contact our office for guidance.

New Patient Forms

Starting your care at Malheur Memorial Health Center Medical Clinic means filling out patient forms at your first visit or your first visit each year. This helps us keep your information current and provide the best care possible. Completing forms ahead of time can help make your visit quicker and easier.
